Fact Sheet for Websterpaks
WHAT WE NEED TO GET STARTED
- A signed and dated medication chart from your usual GP. Please include medications that aren't packed eg puffers, and any over the counter preparations you use.
- All your scripts and repeats paperwork you have to go on our file.
- Any bottle or packets of medication you have at home that we can use for your Webster-pak this saves wastage and the risk of doubling up.
- We need your current address, phone number, date of birth, health care card or pension card number and expiry and your Medicare number and expiry.
We will establish a pharmacy account for you. This is a 30 day account and failure to pay amounts due will result in a suspension of the Webster-pak service.
Webster-pak have a fixed cost per week. The usual cost of your dispensed medications is charged to your account.
HOW DOES IT WORK
Webster-pak contain 7 days of medication. Monday is the starting day. Webster-pak are ready for collection or delivery the Thursday before.
Please ask about our FREE delivery service.
We require at least ONE weeks notice if you need more than one Webster-pak at time (eg holidays). We would ask you to let us know as soon as possible if you are unexpectedly away (eg hospital).
IMPORTANT LEGAL REQUIREMENT
Any change in medication ordered by the Doctor must have written or verbal notification from the Doctor. Please take your Webster-pak with you to Doctor or specialist visits.
Please ask one of our pharmacists for a demonstration on using the Webster-pak There are aids to help you if you have trouble handling the packs.
From time to time we may attach a note to your Webster-pak asking you to arrange for new prescriptions. Please do this immediately as we are unable to dispense medications to go in your Webster-pak unless we have prescriptions.
